Physician Health Partners (PHP) is happy to release a request for proposal from interested home health or home-based therapy providers to provide services in the Denver metro area. PHP and its two client independent practice associations, Primary Physician Partners and South Metro Primary Care, seek proposals to help improve the coordination and quality of healthcare for nearly 50,000 Medicare fee-for-service and Medicare Advantage patients.
