Physician Health Partners currently works with two large adult Independent Practice Associations (IPAs) in the Denver metro area — Primary Physician Partners (PPP) and South Metro Primary Care (SMPC). Currently there are more than 200 primary care physicians combined who practice family medicine, geriatric medicine, or internal medicine. These PCPs serve more than 420,000 Denver metro area patients. PHP supports the IPAs with management, contracting support and network development, provider relations, care coordination, financial and data management, patient care coordination, risk adjustment coding education, and clinical quality improvement services.

On January 1, 2016, PPP and SMPC joined forces as PHPprime, with a combined total of more than 300 participating physicians and midlevel providers. Working together as one contracting entity provides an increased opportunity to improve care for their patients and create innovative solutions to problems.
