Founded in 1996, Physician Health Partners (PHP) is a management services organization that believes physicians and
other providers have the capability and responsibility to drive high quality, cost-effective healthcare for patients.

effective patient care throughout the health care continuum.

Our innovative services create solutions for the healthcare market by delivering performance tools and expertise
to improve healthcare quality, reduce overall system costs and increase the clients’ business success. Our
current customers include two adult independent physician associations (IPAs), a pediatric IPA, and three
self-funded government contracts.

At Physician Health Partners (PHP), our success is directly measured by the success of our customers.
Through our robust Practice Quality Department and diverse medical management solutions, PHP works tirelessly
to improve clinical outcomes for the benefit of those we serve: physicians and other providers, their patients,
and the payors. By improving clinical outcomes, enhancing provider performance and developing practice
efficiencies; patients’ health status is improved and overall healthcare costs are reduced.
Our Customers
PHP was founded in 1996 to serve the largest Primary Care Independent Practice Association (IPA) in Denver. Primary Physician Partners (PPP) has approximately 150 family practice, internal medicine, and geriatric physicians, generally in northwest Denver and has been consistently successful in managing the care of their patients with high quality outcomes. PHP also supports Colorado Pediatric Partners (CPP), a multi-specialty pediatric IPA whose physicians serve children and their families throughout Denver. In addition, PHP supports South Metro Primary Care (SMPC) , a Primary Care Independent Physician Association (IPA) comprised of over 40 Family Practice and Internal Medicine physicians, located in southwest Denver. The newest addition to our customer group is KEY Primary Care Physicians (KEYPCP), an Independent Practice Association (IPA) comprised of Internal Medicine and Family Practice physicians from the east and south metropolitan Denver area that organized in early 2008.
PHP’s Correctional Health Partners Division serves the Colorado
Department of Corrections, the Colorado Division of Youth Corrections, the Colorado Mental Health Institute at Fort Logan, and Oregon Department of Corrections. PHP works closely with each contract to deliver and coordinate the care of individuals when it is not possible to care for them within State facilities.

PHP’s services are tailored to each of our customers. Our experienced, creative healthcare professionals
adapt our services of claims, utilization management, contracting, disease management, and others to meet
the unique needs of our customers. PHP’s structure allows us to be innovative in our approach to meet our
clients’ needs. We listen, we make sure we know our customers’ day to day realities, and we collaborate
with our clients and providers to design our services to meet their requirements.
