Colorado Community Health Alliance (Medicaid)

Colorado Community Health Alliance (CCHA) is a partnership between Physician Health Partners (PHP), Centura Health and Primary Physician Partners. CCHA collaborates with key community resources, medical management and hundreds of primary care physicians to provide Coloradans with dedicated medical homes and quality health care options. CCHA is committed to providing the right care, at the right time and in the right setting.

In early 2011, CCHA was awarded a Medicaid contract from the State of Colorado. CCHA is the Regional Care Collaborative Organization for Region 6 as part of the Accountable Care Collaborative (ACC) Program. Region 6 covers Jefferson, Boulder, Broomfield, Gilpin and Clear Creek counties, and there are currently more than 126,000 ACC members assigned to CCHA.

PHP manages the Medicaid contract and houses the physician network management, contract management, care coordination, quality improvement, marketing/communications, and community resource management.
